Public Relations Release Form
Permission for the Crew to use Crew Member names & pictures (MS Word).

Local Tour Permit
Required for trips and camps under 500 miles.

National Tour Permit
Required for all trips 500 miles or more one way from home area or crossing national boundaries into the territory of other nations.

Class 1 and 2 Medical Forms
Class 1 is required to be filed annually and is required for programs not exceeding 72 hours.
Class 2 is required to be filed every 36 months and is required for programs lasting longer than 72 hours.

Class 3 Medical Form
Class 3 is required every 12 months for high adventure activities, athletic competition and world jamborees.

Unit Money Earning Application
Required for all non-BSA fundraisers.

Northern New Jersey Council Camp Reservation Form

Required for camping at any NNJC facility
